葉家大小姐葉欣瑤在和聶雲川結婚前晚被同父異母的妹妹葉欣月所害，並被妹妹搶走生母留下的唯一遺物“千機盒”。所幸葉欣瑤被醫仙苦大師所救。三年後，葉欣瑤“重生”歸來，帶着醫仙醫術真傳和天龍會萬億財富，大鬧葉欣月和聶雲川婚禮現場，並結識城中首富陸皓軒。葉欣瑤爲奪回母親遺物，施展各種馬甲能力，找出“千機盒”的祕密，並最終將繼母錢麗珠、妹妹葉欣月、妹夫聶雲川趕出葉家，自己成爲了葉家掌舵人，同時還找到真心相愛的伴侶陸皓軒，和其共結連理。
